Regional Hospitals primary objective is to ensure high quality health
care and cost effective outcomes by providing inpatient hospital,
inpatient rehabilitation, outpatient surgical, diagnostic and
rehabilitation services. We are currently looking to fill the position
of full-time RN, Case Manager.
Position Summary: Provides discharge planning and care coordination to
facilitate the delivery of cost-effective quality healthcare. Assists in
the identification of appropriate utilization of resources across the
continuum of care. Works collaboratively with interdisciplinary staff
members.
Position Requirements:
Registered Nurse preferred or LPN with 3-5 years experience in Admissions/UR Setting required.
Current unrestricted RN or LPN License from the State of Michigan required.
