Full time licensed social worker needed for skilled nursing facility.
Applicant must have experience in long term care, be licensed in the State of Michigan, proficient with MDS 3.0, care planning, behavior management, working with ancillary providers, residents, family members and staff.
Applicant must be able to successfully complete assessments and dishcarge plans timely. Communication and documentation skills are a must for this position.
Social worker position is available on a short term rehabilitation unit in the nursing facility.
Salary: $20.00 - $25.00 an hour
