Man missing after leaving NW Side hospital

Police are asking the public's help to find a man who went missing after he left a Northwest Side hospital Tuesday afternoon.

Stephen Ustaszewski, 56, was last seen about 3 p.m. leaving Presence Our Lady of the Resurrection Medical Center, 5645 W. Addison St., Area North detectives said in a missing person alert.

Ustaszewski has a brain disorder that affects his balance, and uses a walker to assist him, the alert said.

He is described as a 5-foot-10, 160-pound white man with brown and gray hair, blue eyes and a light complexion, the alert said. He was last seen wearing a tan jacket, a blue sweater, blue jeans a blue hat and black shoes.
